New Patients:
Please arrive 20 minutes early for your first appointment to fill out the necessary paperwork which includes a thorough health history.
If you prefer, you can download the forms and fill them out in advance to save time.

24 hour Cancellation Policy :
If you have to cancel or reschedule your appointment, please remember to give us a minimum of 24 hour notice to avoid the $35.00 cancellation charge.

Payment Methods
Full payment is due at the time of service. Payments by cash, check, Visa and MasterCard are gladly accepted.

Insurance
We currently accept major insurance plans that offer acupuncture coverage out of network benefits.  
We will gladly check your insurance coverage for you.
You are responsible for your deductible, your co-pay and co-insurance amount.
Payment is due when services are rendered .

Please note: acupuncture is usually covered but we strongly urge you to confirm your coverage with your health insurance carrier prior to treatment to avoid any unpleasant surprises.

Confidentiality
Patient confidentiality will be strictly maintained at all times as required by law.
